Effective January 3rd, 2022, anyone entering a First Ascent Chicago facility will be required to provide proof of vaccination.
Use the form below to submit your proof of vaccination digitally. Once you’ve provided proof of vaccination, we will maintain a record of your vaccination status so that you no longer need to share your vaccination proof for each visit (please bring your proof to the gym until you receive a confirmation that we have update your account). If you do not wish for us to record your vaccination status, you may show your proof of vaccination to front desk staff each time you visit the gym.
